Patients in Whom to Avoid NOACs

Avoid Novel Oral Anticoagulants (NOACs) in the following groups of patients:

  • severe liver impairment – Child Pugh C
  • renal impairment – Creatinine Clearance (CrCl) < 30 mL / min
  • Pregnancy (warfarin is also contraindicated)
  • Breastfeeding – warfarin is ok
  • Co-administration with strong enzyme / transporter-
    • inducers: e.g. rifampicin
    • inhibitors: e.g. erythromycin
  • established warfarin treatment with high percentage of time (> 70%) in therapeutic range
