Avoid Novel Oral Anticoagulants (NOACs) in the following groups of patients:
- severe liver impairment – Child Pugh C
- renal impairment – Creatinine Clearance (CrCl) < 30 mL / min
- Pregnancy (warfarin is also contraindicated)
- Breastfeeding – warfarin is ok
- Co-administration with strong enzyme / transporter-
- inducers: e.g. rifampicin
- inhibitors: e.g. erythromycin
- established warfarin treatment with high percentage of time (> 70%) in therapeutic range
